To be cleared for laser vision correction surgery, please provide the following tests, which are mandatory for elective outpatient surgery:
— Blood test for hepatitis B and C virus antigens;
Note: valid for 3 months.
To be cleared for outpatient cataract phacoemulsification surgery, please provide the following tests, which are mandatory for elective outpatient surgery:
— Blood test for hepatitis B virus antigen;
— Blood test for hepatitis C virus antigen;
— Blood test for HIV 1/2 antibodies and p24 antigen.
Note: valid for 3 months.
Important! If you have chronic conditions (hypertension, diabetes, coronary artery disease, etc.) and are under the care of a specialist (therapist, endocrinologist, cardiologist, or other relevant specialist), you must strictly follow their prescribed treatment and, if required by your attending physician, bring a report from the relevant specialist.
Important!!! The validity of the provided information must be confirmed by the seal of the medical facility, indicating the date and test number.
In accordance with the Resolution of the Government of the Republic of Kazakhstan No. 1241 of October 31, 2011, if you need to obtain a sick leave certificate, you must appear in advance at your local polyclinic and present an extract from the ASTANA VISION outpatient record indicating the recommended elective treatment (surgery) and the scheduled date of this treatment (surgery). For out‑of‑town patients referred to medical organisations outside their permanent place of residence, the sick leave certificate is issued by the referring medical organisation based on the VKK (Medical Advisory Commission) conclusion for the required number of calendar days necessary for travel, but not exceeding five calendar days (Section 2, Clause 26).
Please note that treatment is not contraindicated in the absence of acute or chronic diseases in the acute stage.
